The Utah TBI Plan supports the development of a comprehensive, coordinated system of care for individuals with TBI.

If you are an Individual with a Traumatic Brain Injury complete the “TBI Needs Assessment - Individual with TBI”, with or without help of another person.

If you are a spouse, family member, caregiver, or significant other of an individual with a Traumatic Brain Injury complete the “TBI Needs Assessment - Family/Significant Other”.

Your completed survey will be submitted electronically to the Utah Traumatic Brain Injury Advisory Board, Utah Division of Services for People with Disabilities.  All information is confidential.  Thank you for your HELP.

If you cannot access these surveys using the links provided, or experience technical difficulties completing the survey, email Breanne Berg at bcberg@utah.gov and request an “individual” and/or “family” survey by mail.
